A 42-year-old Canadian politician, serving as the head of his political party, was referred to the Bochner Eye Institute for refractive surgery evaluation. He reported longstanding contact lens intolerance and increasing frustration with his dependence on glasses, particularly during political campaigns and routine daily activities.
PRE-OPERATIVE ASSESSMENT:
Manifest Refraction:
- Right Eye: +2.00 -1.00 × 65 (BCVA 20/20)
- Left Eye: +1.75 -0.75 × 50 (BCVA 20/20)
- Corneal Topography: Normal, with no evidence of irregular astigmatism.
- Corneal Pachymetry:
- OD: 535 µm
- OS: 540 µm
- Fundus Examination: Normal bilaterally.
Given the patient’s hyperopic refractive error, favorable corneal thickness and normal topography, both LASIK and refractive lens exchange (RLE) were discussed as viable surgical options.
MONOVISION TRIAL
An in-office contact lens simulation was performed to evaluate monovision tolerance. The dominant right eye was targeted for full distance correction, while the non-dominant left eye was trialed with a -1.00 D near target. SURGICAL INTERVENTION
